Many small sellers take orders from Instagram posts and WhatsApp chats: a customer sends a screenshot, the seller asks for size, colour and address, and then the parcel comes back because nobody picked up. This form takes the product code, size, colour and address in one go, and asks cash on delivery customers to confirm they will accept the parcel.

In practice
A seller posts a kurta on Instagram with the product code in the caption and the form link in her bio. A customer enters the code, picks M and green, and chooses cash on delivery after ticking the confirmation. The seller books the courier from the dashboard.
Not a fit if
Cash on delivery parcels are refused more often than prepaid ones, which costs you shipping both ways. A confirmation step before dispatch filters out casual orders.
Yes. Say it in the form description, and the form takes the payment screenshot.
The form takes one product code per submission. Ask repeat customers to submit again, or edit the form to take a list.
