Most Bhai Dooj orders are not for the person placing them. A sister in Pune orders a thali and sweets for a brother in Hyderabad, and the seller needs the recipient's phone number, address and a firm deliver-by date, not just the buyer's details. This template is built around that sender and recipient split, so you are not chasing missing addresses on WhatsApp in the days after Diwali.

In practice
A sweet shop shares this link on WhatsApp Status the week after Diwali. A customer in Pune picks a tilak thali and sweets combo, enters her brother's name, phone and Hyderabad address, adds a tilak kit, and sets the deliver-by date. The owner filters orders by city to batch courier pickups.
Not a fit if
Yes. Bhai Dooj follows Diwali by a couple of days, so many shops send one link for the Diwali rush and a second for Bhai Dooj gifts. Keep them as separate forms so the two festivals' orders do not mix in one dashboard.
Put your last courier date in the form description and set a response limit. For other cities, quote a cutoff a few days earlier than for local delivery.
This template uses a UPI payment screenshot upload so respondents can submit without paying first. To have payment verified automatically and settled directly to you instead, swap it for a Payment field connected to your own Razorpay account.
